    For members of the FondaMental Network who wish to access data from the FACE database, here is the procedure to follow: 1. Fill out a summary and reference request form from the FACE database and send it to face@fondation-fondamental.org. 2. Following this submission, two documents will be sent to you: the FACE reference list (list of all the available variables) and the FACE summary (data classified by major themes) specific to the specified pathology. 3. To make an extraction request, the complete dossier to provide includes the three following elements: - the FACE data extraction request form (completed and signed) - the letter of commitment (completed and signed) -the reference list (with the requested variables ticked) All these documents should be sent to: face@fondation-fondamental.org
