Fill out a summary and reference request form from the FACE database and send it to face@fondation-fondamental.org. 2. Following this submission, two documents will be sent to you: the FACE reference list (list of all the available variables) and the FACE summary (data classified by major themes) specific to the specified pathology. 3.
